Understanding Camp Forms

At Camp Nageela, the safety of your child is our number one priority

This is accomplished through the joint partnership of our parents and the care of our talented and experienced team of staff. Our online Campminder Parent Service Center portal has a number of forms for parents to submit before the summer, the first step towards your child having a safe and healthy experience while at camp nageela. No camper will be granted an admission card or allowed into camp until all forms have been completed.

IMPORTANT! If you have multiple children enrolled in camp, you will need to submit a separate set of all forms for each child.

General Forms

This section explains all general camp forms.
Medical related forms can be found in the next section below.

Medical Forms

We want to reiterate that all medical forms must be sent into our office before camp begins. We will not issue admission cards to campers who have not sent in completed medical forms. Please plan for yourself ample time to have your child’s doctor properly fill out the form as the process may be lengthy.

There are six medical related forms that MUST be completed

  1.  Health History form to be completed and signed by the camper’s parent / legal guardian
  2.  Physician’s Exam form to be completed and signed by a doctor
  3.  Mental Health Questionnaire Form to be completed and signed by the camper’s parent / legal guardian
  4.  Insurance Card form to be completed and submitted by the camper’s parent / legal guardian
  5.  Immunization Record Form to be signed by a doctor or on a doctor's official letterhead
  6.  Meningitis Vaccination Response Form to be completed and signed by the camper’s parent / legal guardian

Why these forms are essential

As you are aware, during the camp season children are more active than they are during the year. Although we try to be as safe as possible, injuries or health issues may arise. When an issue does arise, it is imperative that we have current, up to date information about your child’s health history so we can give them the proper care they need.

It is extremely important and legally required that these forms be completed, signed and submitted to the camp before the start of the camp session. These forms authorize the camp to administer necessary medical care when the need arises. We would be unable to care for your child without these forms. No camper will be admitted into camp without these forms!
